I, the undersigned, certify that I am the legal owner or authorized agent of the owner of the pet described above. I hereby authorize Friend to Fern to perform terramation services for my deceased pet.
Terms and Conditions
1. Service Description: I understand that terramation (natural organic reduction) is a process that transforms remains into soil through natural decomposition.
2. Timeline: I understand that the terramation process typically takes 10-12 weeks to complete.
3. Return of Soil: I elect to:
4. Personal Items: I understand that non-organic items cannot undergo the terramation process and must be removed prior to the procedure.
5. Identification: I understand that my pet will be identified throughout the process, and I will receive the soil produced specifically from my pet’s remains.
6. Payment: I understand that full payment is due at the time of service arrangement.
7. Hold Harmless: I hereby release, indemnify, and hold harmless Friend to Fern from and against any and all claims arising out of or resulting from the services provided.
8. Cancellation Policy: I understand that if I cancel the service after it has begun, I am responsible for paying all incurred costs.
9. Bone Remains: I understand that bone remains will be pulverized into small pieces and returned to the composting organic matter. Even with the exercise of reasonable care and the use of the Composter’s best efforts, it is not possible to recover all particles of the composted remains of the Deceased. Some particles may inadvertently become commingled with particles of other composted remains remaining in the chamber and/or other devices utilized to process the remains. I hereby authorize the Composter to dispose of any such residual particles in any lawful manner it deems appropriate.
